All five formats share the same encrypted column space under the hood. Switching format isn't a migration, it's a setting. Practices with mixed clinical styles run multiple formats side-by-side without conflict.

How drafting works

Typed transcript

Paste your session notes, dictation transcript, or a free-form summary. Praxnote drafts from whatever shape you give it.

Uploaded audio

Drop in a phone recording or audio file. Praxnote transcribes locally before drafting; raw audio never leaves your encrypted store.

Telehealth recording

For practices using a supported telehealth integration, Praxnote drafts directly from the post-session recording, with consent on the chart.

Plan divergence

Praxnote checks the draft against the active treatment plan and surfaces deviations inline. You decide whether each is intentional clinical movement or worth flagging.

Versioning & signature

Every signed note is locked. Every prior version is kept.

Praxnote keeps multi-version history. Edit freely before signing, every change rolls into the working version. When you sign, that version becomes immutable, and the signature event is recorded in the activity log.
